We Accept Insurance!
Let Us Check Your Coverage
Our office can determine if our services would be covered by your particular insurance policy. We have successfully processed claims from insurance companies such as United, Cigna, Aetna, and Blue Cross/Blue Shield, to name a few. We are a participating provider with Blue Cross Blue Shield and United Healthcare. We can bill other plans as long as they're not an HMO.
Should your policy be ineligible, we offer discounted rates for payment at time of the office visit. Please call or email us to discuss a self-pay treatment plan.
We accept only checks and cash for payment.
Privacy Policy
By submitting this form, I understand that my personal information will be used initially for the insurance verification process. Once a scheduled consultation and treatment has been conducted, your information may be used for billing your insurance company for payment of services rendered. The information will be accessible to Healthpointe Acupuncture staff and to a third-party biller. I understand that I have the right to request restrictions with the use and disclosure of my health information.
